You must supply a copy of the patient’s driver’s licence (with signature) or passport (with signature) or Aged Pension or Healthcare Card (with signature)
together with a signed consent form to release their medical records. Send via email ONLY.

Patients: You must supply copy of your Medicare card and a copy of your drivers licence with signature (both sides) or a copy of your passport (with signature) or
a copy of your Aged Pension card (with signature) or Healthcare card (with signature). Send via email ONLY.
